Location: Guildford (with travel across Surrey)
Working pattern: Monday to Friday, 9.00am to 5.00pm
About the role
Catalyst in partnership with Surrey and Borders Partnership Foundation Trust and Barnardo’s are providing Liaison and Diversion services to the Surrey community.
The aim of the Reconnect service is to provide care after custody for people leaving prison.
Surrey will be utilising its Liaison and Diversion Service (L&D) contract to deliver Reconnect services. With the L&D service, people of all ages passing through the criminal justice system are assessed and those with mental health, learning disability, substance misuse and other vulnerabilities are identified as soon as possible in the justice pathway. Identified individuals are provided with and supported to access appropriate services including, but not limited to, mental and physical health care, social care, substance misuse treatment and safeguarding.
Reconnect services in Surrey aim to build upon the L&D model to have a whole system approach to include people leaving custodial settings so that they continue to receive support in the community.
Reconnect Assertive Outreach Workers will need to be vetted to attend prisons to carry out assessments of needs prior to their release. You will be working over six prisons, three male, three female and in the community. You will spend most of your time in the community however there is expectation to attend the prisons to complete assessments and visits ready for release.
You will work as part of a team as well as independently holding your own diary seeing clients regularly for support.
