Community Connections – Primary Care Link Worker   North West Surrey

Development Role for 11 months from April 2019   –  Full-time 35 hours

Exciting opportunity to be involved in a pilot to better integrate physical and mental health and improve people’s access to existing services via a primary mental health care service.  We are looking for someone with real interest in and demonstrable experience of working with people with a range of mental health needs and who is passionate about enabling them to be supported and access the best care and support.

Community Drug and Alcohol Worker – part time 3 days/ 21 hours  Guildford

Catalyst is looking for an individual with experience in working with people whose drug and or alcohol use is affecting their health and wellbeing. The role involves working within a multi-disciplinary team based in Guildford alongside our local NHS partner to provide a seamless service and a broad range of interventions, treatment and recovery support for people with alcohol dependence, and/or problematic drug use.
