A chance to be involved in a special project working to have a positive impact on rough sleeping in Guildford, with particular emphasis on people with substance use issues.

Are you:

  • skilled in providing support within a multi skilled/disciplinary approach within teams,
  • able to engage and interact with people in challenging circumstances, and support people to stabilise their behaviours and improve their quality of life?

Co-located with the Rough Sleeper Outreach Team in Guildford town centre, the Substance Misuse Outreach (SMO) Navigator will be working alongside colleagues including a  Mental Health Social Worker.

The Navigator would work with people who have substance misuse issues and who are:

  • Current rough sleepers
  • Recent rough sleepers placed in short term accommodation
  • Former rough sleepers at imminent risk of returning to rough sleeping.

You will work within a multi-disciplinary team based in Redhill, alongside our local NHS partner, to provide a seamless service and a broad range of interventions, treatment and recovery support for people with alcohol dependence, and/or problematic drug use. Your role will be to respond to individual needs and could include the following; advice, information, harm reduction, needle exchange, assessment and care planning, one-to-one work using evidenced based approaches and group work.

Our aim is to help individuals, alongside their families and carers, achieve and sustain their personal goals and improve their health and wellbeing for their future.

Whilst experience of drug and alcohol support work is highly desirable, we welcome applications from candidates with experience of supporting vulnerable clients in a variety of settings.  This is a chance for you to broaden your skillset and receive training, development and mentoring in this area of work.

Catalyst High Impact (CHI) Assertive Outreach Worker – Surrey-wide

Full or Part-time

Catalyst is looking to appoint two people who have experience of assertive outreach to work within a small team which works closely with Alcohol Specialist Nurses and Community Harm and Risk Management Meetings (CHaRMMs) to work differently with drinkers not in contact with treatment services and who have complex needs.

The Catalyst High Impact team work to the approaches of the ‘Blue Light Project’ providing engagement, harm-reduction, motivation to change, and preparation for treatment for high risk and alcohol dependent drinkers who are having the greatest impact on public services and the wider community across Surrey.
